There is more to ‘tears’ than meets the eye

It’s better to shed tears than having a ‘Dry Eye’. Literally, because, it’s actually a medical condition that occurs when eyes do not produce tears properly. It also makes working on a computer, reading for a longer stretch of time difficult. It is therefore advisable to protest ourselves from this condition that causes burning sensation in the eye, watering, blurring of vision, white discharge from the eyes, sticky eyes, red puffy eyes and sometimes agonizing pain in the eyes.

Dr Suvarna Koppikar (MS Ophthalmology), a renowned eye-surgeon from Mumbai, Maharashtra, take us through the mysterious world of ‘tears’ and all about the Dry-Eye syndrome.

Tears are equated with grief, and sometimes with joy. Do tears play any other role?
A big role, they are the pillars of our eye-health. Tears are essential for clear and comfortable vision. Any disturbance in the quality of tears or consistency of tears is termed as ‘dry-eyes’.

Tears are just salt water, aren’t they?
Far from it. Those tiny droplets are actually made of three layers – an oily layer, a watery layer and a mucus layer. The fabric of tears is vital. Each of these layers has their own purpose and when there is disturbance in formation of any one of these layers it is called as dry eyes.

Tell us more about these ‘layers of tears’
The oily or lipid-layer (outermost) is produced by glands which are present in the eyelids. Its purpose is to keep the tear surface smooth and reduce or prevent evaporation of tears. The middle watery-layer makes up most of what we ordinarily think as tears. This layer is produced by the lacrimal glands in the eyelids to cleanse the eye and wash away foreign particles or irritants that try to enter our eyes as the surface of our eye remains exposed to the outer environment. The inner layer is made up of mucus which is produced by the white surface of the eye called conjunctiva. Mucus allows the watery layer to spread across evenly over the surface of the eye and helps the eye remain moist. Mucus layer is very important as without mucus tears would not stick to the eye.

When we shed tears of grief or while cutting the onions, we are actually protecting our eyes, right?
Not really. We produce basic tear at a slow and steady rate, these tears do not well up in our eyes or flow out like when we cry. Basic tears constantly bathe our eyes. The eyes stay moist and comfortable because of basic tears. When we cry due to emotions or while cutting the onions, we shed what is called the reflex tears.

Do you mean that we suffer from ‘dry eyes’ when we stop producing the basic tears?
Dry eyes do not mean no tears or less tears but it is the disturbance of quantity or quality tears. People with dry eyes complain of “WATERING ‘of eyes which disturbs them.

What are the causes of ‘dry-eyes’?
Dry-eyes can be due to local, internal, environmental or life-style related factors. Local causes pertaining to the eye are long term use of contact lenses and eye-drops that contain preservatives. Internal problems especially auto immune diseases (when our immune cells attack our own body) like rheumatoid arthritis or diseases like diabetes and Herpes Zoster (RASH touching the eyes) lead to reduced tear secretion and thereby cause dry-eyes. Many common medications like painkillers, anti-anxiety medications, sleeping pills, anti-allergic, and certain blood pressure medicines can lead to dry-eyes as side effects. Environmental factors like allergies related to seasonal changes and air pollution are common in children and give rise to short term dry-eye symptoms.

How about artificial tears? Do they also have preservatives? Any other reasons for ‘dry-eyes’?
Preservative free artificial tear eye drops are now available in the market as daily disposable units. Other common causes of dry eyes are the surrounding environment. Excessive dry or windy climate as well as air conditioning which evaporate tears gives rise to “evaporative dry eyes”. Avoiding these irritants and using protective glasses can give relief to dry eye symptoms.

Are ‘dry-eyes’ more common in men or women?
Anyone can experience dry-eyes symptoms though it is more common in women especially after menopause and other hormone related conditions like pregnancy and menstruation.

What are the lifestyle factors that lead to ‘red-eyes’?
That’s a good question. Normally we blink 18-20 times a minute but studies show that blink rate goes down by half or more while using computers and other digital screen devices either for work or for play. These days children are exposed to computer screens and other devices such as mobile phones etc and they use these devices to play for long hours. They develop eye strain and other dry eye symptoms of watering, gritty sensation, itching and photophobia (sensitivity to light).

Extended reading, writing or other intensive “near-work” can temporarily cause eye strain and watering of eyes or blurring of vision. Computer related symptoms become worse by sleep deprivation. During sleep our eyes are replenished by nutrients and regain the focusing power for close distance activity. Contact lens induced dry-eyes are due to infrequent blinking, improper cleaning of lenses, sleeping with lenses on.

Excessive smoking can also lead to dry eyes. What kind of treatment do you recommend?
Though many a times dry-eyes cannot be cured completely there are a number of steps that can be taken to treat them.
Use of artificial tear drops and ointments is the primary treatment for dry eyes. Preservative free eye drops are better for people with chronic dry eye and it is important to use the drops even when eyes feel fine to keep them lubricated. Lubricant eye ointments are also available for people with chronic dry eye symptoms.

Disposable eye drops containing cyclosporine that helps eyes increase their own tear production with continued use are available and are a boon for chronic dry eye patients.

There is growing evidence that increasing the oral intake of fish oil and omega 3 via diet or supplement is very helpful to those suffering with dry eye.
Self-medication is strongly discouraged as the chemists may dispense eye drops containing variable amount of steroids which have side effects. A formal consultation with an ophthalmologist is recommended to get the diagnosis established.

What about crocodile tears Doc?
Oh they are most dangerous. I recommend no one must shed them!

If you are using contact lenses

* Limiting use of contact lenses and wearing glasses periodically.

* Practice correct method of cleaning your lenses.

* Wash your hands before handling lenses.

* Avoid touching lenses with water.

* Use fresh solution every time for storing and cleaning.

* Rub your contact lenses when you clean them.

* Clean your storage case daily and replace it every 2-3 months.

* Stop wearing lenses if you develop redness, irritation, watering, sensitivity to light, burning, eye swelling or discharge. Do not take these symptoms lightly. One must visit an eye doctor if you develop these symptoms.

To reduce computer related eye strain and dry eyes:
* Sit about 25 inches from the computer screen and position the screen so that your eye gaze is slightly downward.

* Practice the rule of 20. Every 20 minutes shift your eyes to look at an object 20 feet away for at least 20 seconds.

* Use artificial tear eye drops to refresh your eyes when they feel dry.

* Blink often and practice conscious blinking i.e. remind yourselves to blink.

* Take regular breaks from computer work and try to get enough sleep at night.

Sodas, fruit drinks and other sugary beverages may lead to an estimated 184,000 adult deaths each year worldwide from diabetes, heart disease and cancers, researchers, including one of Indian-origin, have found.

“Many countries in the world have a significant number of deaths occurring from a single dietary factor, sugar-sweetened beverages. It should be a global priority to substantially reduce or eliminate sugar-sweetened beverages from the diet,” said Dariush Mozaffarian, senior author of the study from Tufts University in Boston.

In the first detailed global report on the impact of sugar-sweetened beverages, researchers estimated deaths and disabilities from diabetes, heart disease and cancers in 2010.

In this analysis, sugar sweetened beverages were defined as any sugar-sweetened sodas, fruit drinks, sports/energy drinks, sweetened iced teas, or homemade sugary drinks such as frescas, that contained at least 50 kcal per 8 ounces serving. 100 per cent fruit juice was excluded.

Estimates of consumption were made from 62 dietary surveys including 611,971 individuals conducted between 1980 and 2010 across 51 countries, along with data on national availability of sugar in 187 countries and other information.

Based on meta-analyses of other published evidence on health harms of sugar-sweetened beverages, researchers calculated the direct impact on diabetes and obesity-related effects on cardiovascular disease, diabetes and cancer.

In 2010, the researchers estimate that sugar-sweetened beverages consumption may have been responsible for approximately 133,000 deaths from diabetes, 45,000 deaths from cardiovascular disease, 6,450 deaths from cancer.

“There are no health benefits from sugar-sweetened beverages, and the potential impact of reducing consumption is saving tens of thousands of deaths each year,” Mozaffarian said.

The impact of sugar-sweetened beverages varied greatly between populations. At the extremes, the estimated percentage of deaths was less than 1 per cent in Japanese over 65 years old, but 30 per cent in Mexican adults younger than 45.

Of the 20 most populous countries, Mexico had the highest death rate attributable to sugar-sweetened beverages with an estimated 405 deaths per million adults (24,000 total deaths) and the US ranked second with an estimated 125 deaths per million adults (25,000 total deaths).

About 76 per cent of the estimated sugar-sweetened beverage-related deaths occurred in low- or middle-income countries.

“Among the 20 countries with the highest estimated sugar-sweetened beverage-related deaths, at least 8 were in Latin America and the Caribbean, reflecting the high intakes in that region of the world,” said Gitanjali Singh, lead author of the study and a research assistant professor at the Friedman School at Tufts.

Overall, in younger adults, the per cent of chronic disease attributed to sugar-sweetened beverages was higher than the per cent in older adults.

The study was published in the journal Circulation.

Testing on human skin cell lines, this study shows the action spectrum of ultraviolet damage in cells derived from both the upper layer (dermis) and lower layer (epidermis) of the skin.

Our skin ages due to the constant exposure to sunlight as UV radiation comprising UVA and UVB rays from the sun penetrate cells and increase the number of damaging free radicals, especially the reactive oxygen species.

Too many reactive oxygen species can be harmful because they can damage the DNA within our cells.

Over time, this can lead to the accumulation of DNA damage, particularly in mitochondria – the batteries of the cells – which speed up ageing and destroy the skin’s supportive fibres, collagen and elastin, leading to wrinkles.

“Because we were able to analyse the full spectrum of UVA and UVB induced sunburnt DNA damage in the batteries of human skin cells this is an invaluable tool for the cosmetic and pharmaceutical industries and for anti-ageing studies,” said professor Mark Birch-Machin from Newcastle University, England.

Studies strongly suggest the damage caused by reactive oxygen species may also initiate and exacerbate the development of skin cancers.

The results would pave the way for better and more effective sunscreen lotions, the researchers said.

The results were published in The Society for Investigative Dermatology journal.

If you are having sleepless nights, blame it on calcium deficiency as a key calcium channel has been identified as responsible for deep sleep, says new study.

The study also gives us a clue to understanding both normal and abnormal waking brain functions.

“It is the same brain, same neurons and similar requirements for oxygen and so on. So what is the difference between these two states?” asked Rodolfo Llinas, a professor of neuroscience at New York University School of Medicine and a Whitman Center Investigator at the Marine Biological Laboratory (MBL) in Woods Hole.

To tackle the broad question of sleep, Llinas and his colleagues focused on one crucial part of the puzzle in mice, Marine Biological Laboratory.

Calcium channels, selective gates in neuron walls, are integral in neuron firing, ensuring that all parts of the brain keep talking to one other. But during sleep, calcium channel activity is increased, keeping a slow rhythm that is different from patterns found during wakefulness.

Based on this clue, the scientists removed one type of calcium channel, Cav3.1, and looked at how the absence of that channel’s activity affected mouse brain function.

This calcium channel turns out to be a key player in normal sleep. The mice without working Cav3.1 calcium channels took longer to fall asleep than normal mice, and stayed asleep for much shorter periods.

Their brain activity was also abnormal, more like normal wakefulness than sleep. Most importantly, these mice never reached deep, slow-wave sleep.

“This means that we have discovered that Cav3.1 is the channel that ultimately supports deep sleep,” Llinas said.

Because these mice completely lack the ability to sleep deeply, they eventually express a syndrome similar to psychiatric disorders in humans.

The findings appeared in the Proceedings of the National Academy of Sciences.

The team from the University of Southampton found that people with a history of heart disease had substantially lower bone mineral density in their wrist bone than those without.

The technique was used on 350 men and women aged 70-85 suffering from coronary heart disease such as angina, heart attack or heart failure.

The results showed that bone mineral density was lower among participants with coronary heart disease.

The effect was more prominent in women than in men.

“The results highlight the need to evaluate a history of heart disease in the management of osteoporosis in older people,” said professor Cyrus Cooper, professor of rheumatology at the University of Southampton.

Further research is needed to provide a better understanding of the underlying mechanisms which explain the link between osteoporosis and heart disease, the team said.

The results were published in the journal Osteoporosis International.

San Diego State University researchers analysed almost 5000 individuals’ trans fats intake, where 1699 were men and 3293 women, and then examined their emotion responses.

It was observed that individuals with higher intakes of trans fats experienced “difficulties with emotional awareness,” as well as a lower level of emotional “clarity”.

In turn, those individuals with a lower trans fats intake were associated with “increased positive and decreased negative affects” and were better able to control their emotions.

Consuming a diet rich in trans fats has been linked to high cholesterol levels in the blood, causing heart attacks, heart disease and strokes, however, the mental health dangers have not been so widely accepted.

Oxford researcher and head of charity Food and Behaviour Research, Dr Alex Richardson, admitted to the Guardian that they knew such a diet was “wrecking our bodies and brains,” and there was a need look at the totality of evidence instead of “randomised controlled double-blind placebo trials.”

The study is published in the Journal of Health Psychology.

A technique to increase the blood flow from the umbilical cord into the infant’s circulatory system improves blood pressure and red blood cell levels in premature infants delivered by Caesarean section, shows a study by Indian-origin researchers.

The technique, known as cord milking, consists of encircling the cord with the thumb and forefinger, gently squeezing, and slowly pushing the blood through the cord to the infant’s abdomen.

For infants delivered by Caesarean section, cord milking appears to offer benefits over the standard practice of waiting 45 to 60 seconds before clamping and then cutting the umbilical cord.

“The study results are very encouraging,” said Tonse Raju, chief of National Institute of Child Health and Human Development, NICHD’s pregnancy and perinatology branch.

“The results need to be confirmed in a larger number of births, but it appears that umbilical cord milking may prove to be of great benefit to preterm infants delivered via Caesarean.”

The team enrolled 197 infants, of which 154 were delivered by Caesarean, with 75 assigned at random to the umbilical cord milking group and 79 assigned to the delayed clamping group.

Of the infants undergoing Caesarean deliveries, those in the cord milking group had higher blood flow in the superior vena cava, the large vein carrying blood from the brain to the heart.

First author Anup C. Katheria, another Indian-American neonatologist at the Neonatal Research Institute at the Sharp Mary Birch Hospital, noted that some studies failed to find a reduction in intraventricular haemorrhage from delayed cord clamping among preterm infants delivered by Caesarean.

Among preterm infants, the delay is believed to protect from intraventricular haemorrhage, or bleeding in the ventricles — the cavities inside the infant’s brain.

The haemorrhage is thought to result from low blood pressure, brought on by having too little blood in the circulatory system.

Cord milking might compensate for diminished blood flow through the umbilical cord and increase the amount of blood available to the infant.

The study was published online in Pediatrics.

Indian-origin scientist finds key to early Alzheimer’s diagnosis

According to an Indian-origin scientists, errors on memory and thinking tests may signal Alzheimer’s up to 18 years before the disease can be diagnosed.

“The changes in thinking and memory that precede obvious symptoms of Alzheimer’s disease begin decades before,” said study author Kumar B Rajan with Rush University Medical Center in Chicago.

“While we cannot currently detect such changes in individuals at risk, we were able to observe them among a group of individuals who eventually developed dementia due to Alzheimer’s,” Kumar said.

For the study, 2,125 European-American and African-American people from Chicago with an average age of 73 without Alzheimer’s disease were given tests of memory and thinking skills every three years for 18 years.

Twenty-three per cent of African-Americans and 17 per cent of European-Americans developed Alzheimer’s disease during the study.

Those who scored lower overall on the memory and thinking tests had an increased risk of developing the disease.

During the first year of the study, people with lower test scores were about 10 times more likely to be diagnosed with Alzheimer’s disease than people with higher scores, with the odds increasing by 10 for every standard deviation that the score was lower than the average.

Based on tests completed 13 to 18 years before the final assessments took place, one unit lower in performance of the standardised cognitive test score was associated with an 85 per cent greater risk (relative risk of 1.85) of future dementia.

“A general current concept is that in development of Alzheimer’s disease, certain physical and biologic changes precede memory and thinking impairment. If this is so, then these underlying processes may have a very long duration,” Kumar said.

“Efforts to successfully prevent the disease may well require a better understanding of these processes near middle age,” Kumar said.

From the lab: Effect of yoga on body’s ability to fight back

At the Yoga and Biomechanic Lab, we try to carry out scientific analysis of different body postures , body mechanics and balance, including those postures that are required while performing yoga asanas.The benefits of yoga are very well known. But we were trying to analyse the impact of yoga exercises on particular body functions and its role in tackling specific medical conditions.

The Yoga and Biomechanic Lab at IISc and M S Ramaiah Medical College in Bangalore have been collaborating on this project for a long time. Together, we decided to examine the effect of yoga exercises on the body’s ability to tackle inflammatory diseases, a condition the body slips into while fighting an external injury. For the purpose of our study, we had two groups of people, 109 in each group. One group included people who were regular practitioners of yoga, performing the asanas for at least one hour every day for five years. The subjects were all taken from Dr. Omkar’s Yoga Mandir in Bangalore. The other group comprised of non-yoga performing people.

We made candidates from both groups undertake mild and strenuous exercise. The ’10 metre shuttle walk test’ was our preferred choice of exercise. The candidates had to walk back and forth on a 10 metre long path, slowly increasing their pace. On one day, the participants were asked to walk for about 7 to 8 minutes, and on another day they were made to walk for 11 or 12 minutes, which becomes quite arduous as a subject increases his speed. Blood samples of the candidates were collected just before the exercise, immediately after the mild exercise, and then after the strenuous exercise.

We observed that the yoga-performing group, in general, had lower levels of inflammatory cytokines. These are cell proteins that trigger inflammations in the body. In particular, we were studying the levels of two specific pro-inflammatory cytokines: Tumour Necrosis Factor Alpha and Interleukin-6, that play a role in coronary heart diseases, depression, diabetes, arthritis, osteoporosis, Alzheimer’s disease and periodontal (gum) disease.The rate of increase in the levels of these two cytokines (Tumour Necrosis Factor Alpha and Interleukin-6) with the increase in level of physical activity, was much higher in the non-yoga practising group. This is an indication that regular practice of yoga protects against the rise of the harmful cytokines, when people are exposed to physical stress or unaccustomed physical activity.We also observed that cholesterol, triglyceride and VLDL (very low density lipoprotein) levels were significantly low in the yoga practising group. On the other hand the level of HDL (high density lipoprotein), a good cholesterol, was significantly higher in the group. Thus, members of the yoga group were less likely to develop cardio-metabolic disorders. Our findings can have important implications in the treatment of a variety of diseases including diabetes, heart diseases and even cancer.

Smoking may increase the risk of dying early in pre-menopausal women with breast cancer, a research said.

“Overall, this work is monumental in advising patients about how smoking might affect breast cancer outcome,” said co-author Yuko Minami from Tohoku University Graduate School of Medicine, Sendai, Miyagi, Japan.

This study included 848 patients admitted to a single hospital in Japan from 1997 to 2007. Active or passive smoking status was assessed using a self-administered questionnaire.

The patients were followed until December 31, 2010.

The researchers found that pre-menopausal women who smoked for more than 21.5 years had a 3.1-times higher risk of dying from any cause and a 3.4-times higher risk of dying from breast cancer.

These links were not apparent among post-menopausal women.

The increased risks seen in pre-menopausal women were especially relevant to women whose cancers expressed both the estrogen receptor and the progesterone receptor, the study said.

“Hopefully this paper will serve to reduce the number of breast cancer patients who continue to smoke,” Minami said.

Diet diary: Dairy may not be as bad as you thought

Among the first casualties of a calorie or cholesterol free diet are dairy products. But should those starting these diets sacrifice milk-based products? The jury on the issue seems to be still divided.

While the anti-dairy lobby has several explanations for abandoning it, most scientific evidence and dietary guidelines do recommend low-fat dairy products as part of healthy diets. Besides, apart from its commonly known benefits of bone building and valuable nutrition profile, there is more to this food group than we previously knew.

The milk-based products have become a subject of controversy for various reasons: fears of heart disease, obesity and even cancer. Often those diagnosed with a malignancy are made to believe that giving up dairy would be helpful. Dairy products, however, have been found to be a good source of conjugated- linoleic acid (CLA), a variant of the essential linoleic fatty acid; CLA is believed to inhibit tumor growth.

The essential acid occurs in the rumen of cows and other ruminants and is present in their milk and other dairy products including cheese, yoghurt, butter. It can also be found in the meat of cattle and lamb.

CLA research can be traced back to 1977, when Peter Parodi identified it in cow’s milk. The turning point for CLA came after investigations by Micheal Pariza revealed its tumor inhibiting properties. CLA has also been shown to inhibit cancer by several mechanisms. It is a unique anti-carcinogen which inhibits the growth of a number of human cancer cell lines and suppresses chemically-induced tumor development in laboratory animals. CLA has been found to be beneficial particularly in cancers of colon, ovaries, prostate, breast, blood, bone marrow (leukemia) and skin (melanoma). Intake of 1 per cent CLA in diet has been reported to reduce the breast cancer incidence by nearly 60 per cent. A study reported that CLA enriched butter inhibited rat mammary tumor yield by 53 per cent.

Researchers have also discovered, in animal experiments, that CLA helps in lowering cholesterol, very low density lipoprotein (VLDL), low-density lipoproteins (LDL- bad cholesterol), LDL-HDL ratio, total cholesterol-HDL ratio and triglycerides and reduced arterial plaque build up. CLA also prevents heart diseases by exhibiting its antioxidant activity and inhibiting formation of free radicals. Researchers also suggest that CLA has a tocopherol (Vitamin E) sparing effect. Vitamin E is a powerful antioxidant vitamin.

Some studies on animal research reported reduction of body fat following CLA dietary supplements. Postulated mechanisms suggest that CLA reduces body fat by reducing energy intake, increasing metabolic rate, regulating glucose and fatty acid uptake and improving insulin sensitivity. The benefits of using CLA dietary supplements for reducing body fat, however, have not been consistent and conclusive.

Scientists have found a blood pressure medication that can help ward off relapses by erasing memories in the subconscious that trigger addiction.

The experimental drug, which has already been approved by the US Food and Drug Administration (FDA), has successfully halted cocaine and alcohol addiction in a series of experiments, according to a university release.

According to the study published in the journal Molecularn Psychiatry, the treatment could help ward off relapses by erasing memories in the subconscious that trigger addiction.

The drug represents a massive departure from the viewpoint in the 20th century that drug addicts simply lacked willpower.

The study was led by Hitoshi Morikawa, an associate professor of neuroscience at University of Texas, Austin.

In the experiments, a team of researchers trained rats to create an association between either a black or white room and the use of a certain drug.

When addicted rats were given the option of both rooms, they chose the room associated with their addiction nearly every time.

Today, most experts acknowledge that environmental cues — the people, places, sights and sounds an addict experiences leading up to drug use — are among the primary triggers of relapses.

“The isradipine erased memories that led them to associate a certain room with cocaine or alcohol,” said Morikawa.

The researchers found that blocking these ion channels in brain cells, using isradipine, appears to reverse the rewiring that underlies memories of addiction-associated places.

A treatment based on this latest research, however, would be much more effective, said Morikawa, targeting the associations an addict has with the experience leading up to taking a drug.

“Many addicts want to quit, but their brains are already conditioned. This drug might help the addicted brain become de-addicted,” he said.

One challenge with using isradipine in high doses to treat addiction is that it lowers blood pressure.

So it might be necessary to pair it with other treatments that prevent blood pressure from falling too low.

Smart insulin patch to replace painful jabs for diabetics

A ‘smart’ insulin patch that can detect increases in blood sugar levels and secrete doses of insulin into the bloodstream could replace painful injections for diabetics, scientists say.

Researchers at the University of North Carolina and NC State University, developed the patch – a thin square no bigger than a penny – which is covered with more than one hundred tiny needles, each about the size of an eyelash.

The study, which is published in the Proceedings of the National Academy of Sciences, found that the new, painless
patch could lower blood glucose in a mouse model of type 1 diabetes for up to nine hours.

“We have designed a patch for diabetes that works fast, is easy to use, and is made from nontoxic, biocompatible
materials,” said co-senior author Zhen Gu, a professor in the Joint UNC/NC State Department of Biomedical Engineering.

“The whole system can be personalised to account for a diabetic’s weight and sensitivity to insulin so we could make the smart patch even smarter,” Gu said.

For the study, Gu and his colleagues chose to emulate the body’s natural insulin generators known as beta cells. These versatile cells act both as factories and warehouses, making and storing insulin in tiny sacs called vesicles.

“We constructed artificial vesicles to perform these same functions by using two materials that could easily be found in nature,” said Jiching Yu, a PhD student in Gu’s lab.

The first was hyaluronic acid or HA, a natural substance found in many cosmetics. The second was 2-nitroimidazole or NI, an organic compound used in diagnostics.

The researchers connected the two to create a new molecule, with one end that was water-loving or hydrophilic
and one that was water-fearing or hydrophobic.

A mixture of these molecules self-assembled into a vesicle, much like the coalescing of oil droplets in water, with the hydrophobic ends pointing inward and the hydrophilic ends pointing outward.

The result was millions of bubble-like structures, each 100 times smaller than the width of a human hair. Into each of these vesicles, the researchers inserted a core of solid insulin and enzymes specially designed to sense glucose.

In lab experiments, when blood sugar levels increased, the excess glucose crowded into the artificial vesicles. The
enzymes then converted the glucose into gluconic acid, consuming oxygen all the while.

The resulting lack of oxygen or “hypoxia” made the hydrophobic NI molecules turn hydrophilic, causing the vesicles to rapidly fall apart and send insulin into the bloodstream.

The researchers then decided to incorporate these balls of sugar-sensing, insulin-releasing material into an array of tiny needles.

Gu created these “microneedles” using the same hyaluronic acid that was a chief ingredient of the nanoparticles, only in a more rigid form.

Researchers then arranged more than one hundred of these microneedles on a thin silicon strip to create what looks like a tiny, painless version of a bed of nails.

Doctors in Australia have documented in a prestigious peer-reviewed journal “a new neurological complication of wearing tight jeans” that could be potentially more hazardous than the tingling sensations and numbness in the thigh — the result of “lesions of the lateral cutaneous nerve of the thigh, likely caused by compression of the nerve at the inguinal ligament” — that some people have reported earlier.

The new complication, seen in a 35-year-old woman in Adelaide, was the result of “compartment syndrome” — a condition in which dangerously high pressure builds up in a group of muscles or organs in the body, impeding the flow of blood to and from the ‘compartment’ of tissues — greatly exacerbated by the pair of skinny jeans that she was wearing.

The study, Fashion victim: rhabdomyolysis and bilateral peroneal and tibial neuropathies as a result of squatting in ‘skinny jeans’, reported that the woman, who was helping a family member move homes, had been squatting for many hours emptying cupboards, and had felt her jeans get increasingly tight and uncomfortable.

While walking home that evening, her feet had gone numb, and she had tripped and fallen, and lain immobile for several hours before she was found. Doctors found her lower legs swollen enormously, her ankles and toes could not move, and they had to cut her jeans off. Muscle power at the hips and knees, and renal function were normal.

Investigations showed a very high creatine kinase level, oedema in the lower legs, and a hypoattenuation of the  posterior calf muscles consistent with myonecrosis (destruction or death of muscles/gangrene). There was a conduction block in both the common peroneal nerves.

The woman was given intravenous hydration which improved the oedema and neurological function of her lower limbs, but it was still four days before she was able to walk unaided and could be discharged from hospital.

While prolonged squatting may cause common peroneal neuropathy (damage to the peroneal nerve leading to loss of movement/sensation in the foot and leg), combined common peroneal and tibial neuropathies had not previously been described in association with squatting.

The paper postulates that the peroneal neuropathies were caused by compression between the biceps femoris tendon and fibular head as a result of squatting, while the tibial neuropathies were likely caused by compression of the nerves in the posterior compartment of the calf by oedematous muscles that had undergone ischaemic myonecrosis as a result of squatting. The wearing of skinny jeans had likely potentiated the tibial neuropathies by causing a compartment syndrome as the lower legs swelled.

The number of females reaching the age of 100 years between 1990 and 2013 went up by 50 percent, the results said.

The researchers also found that there was also a 30 percent increase in the number of males reaching 100 during the same period.

“We found a surprising number of 100 year-olds who had no major illnesses. However, as the number of people living to 100 continues to increase, it is very important to understand the evolving health care needs of the oldest old,” said lead author of the study Nisha Hazra from King’s College London.

The study, published in the Journal of the American Geriatrics Society, used electronic health records to examine some of the main age-related chronic illnesses, including diabetes, stroke, arthritis and cancer, as well as old age-related problems, including falls, fractures, dementia and hearing/visual impairments, among 11,084 centenarians.

Whilst far less men reached the age of 100, those that did tended to be healthier, with females more likely to experience multiple chronic illnesses and disabilities such as fractures, incontinence and hearing/visual deterioration than men.

Less life-threatening conditions such as arthritis and other musculoskeletal diseases were also shown to be more prevalent than more serious illnesses such as diabetes and cancer across both men and women in the sample.

Attention wearers of skinny jeans: don’t squat — at least not for long.

Doctors in Australia report that a 35-year-old woman was hospitalized for four days after experiencing muscle damage, swelling, and nerve blockages in her legs after squatting for several hours while wearing tight-fitting denims.

“We were surprised that this patient had such severe damage to her nerves and muscles,” said Dr. Thomas Kimber of the Royal Adelaide Hospital in Australia, in an email.

The patient, who was not identified, spent most of the previous day helping someone move, squatting for long periods while emptying cupboards. Clad in skinny jeans, the woman said they felt increasingly tight and her feet were numb as she walked home, making her trip and fall. Unable to get up, she spent several hours stranded outside before getting to the hospital. Kimber and colleagues published a report about the case online Monday in the Journal of Neurology, Neurosurgery and Psychiatry.

Kimber said tight jeans have previously been reported to cause nerve lesions in the groin but not the kind of nerve problems in the lower leg and severe muscle damage they saw. He said squatting compressed the nerves in the lower leg, reducing the blood supply to the calf muscles and that the skinny jeans worsened the problem.

After being treated for four days — and having her jeans cut off — the woman still had some weakness in her legs but walked out of the hospital and later recovered fully. Kimber doesn’t know if the woman still wears skinny jeans but warned her against the dangers of squatting in them.

“I think it’s the non-stretchy nature of jeans that might be the problem,” Kimber said, noting that tight pants with more elasticity wouldn’t be as dangerous since they wouldn’t squeeze nerves and muscles. He doesn’t wear skinny jeans himself, but not because of the medical risks: “I’m too old to get away with them!”

Today is International Yoga Day and right from the Prime Minister Narendra Modi to India’s common citizens, yoga seems to be the buzz word. Of course these days, in India for many people, Baba Ramdev is synonymous with reviving yoga and there are many who attend his camps and watch his show.

However before Baba Ramdev took to our screens to teach us yoga, guru Dhirendra Brahmachari in the 1970s had promoted Yoga on Doordarshan. His show was a weekly one on DD and he had also introduced yoga as a subject in Kendriya Vidyalayas.

Brahmachari, who was the yoga mentor of late Prime Minister Indira Gandhi, had ashrams in Delhi, Jammu and Kashmir.  The current Moraji Desai National Institute of Yoga was earlier owned by him. Brahmachari died in 1994 in a plane crash.

With upcoming International Yoga Day, experts talk about ways in which this age-old practice can cure the side effects of chemotherapy in various types of cancer and also increases the patient’s life span.

They have urged nations across the world to integrate Yoga with the modern medicine, as research has shown evidence that Yoga supports treatment and helps patients in tackling several dreaded diseases.

“Not just Indian but American and European studies have concluded that Yoga helps in decreasing nausea and vomiting during chemotherapy. It not just minimises the pain and the anxiety in the patients but also helps them increase the toleration power and immunity. However, it is necessary for the patient to continue practicing yoga,” P.K. Jhulka, a renowned oncologist and former dean of the All India Institute of Medical Sciences (AIIMS), told IANS.

Noting that while undergoing chemotherapy, patients suffer from weakness and mental trauma, he said that Yoga gives mental and physical balance to the patient’s life.

Recent researches have proved that Yoga is beneficial for a plethora of 101 health complications like improving lymphatic circulation, lower blood pressure, lubricate joints and ease the pain of scoliosis.

Sunil Mittal, an eminent psychiatrist and chairman of Cosmos Institute of Mental and Behavioral Sciences (CIMBS), said Yoga is the strongest tool for enhancing health and in building self-confidence.

“There have been proofs that patients with continuous Yoga have recovered even failure of body immunity. It is a holistic science that improves body strength and helps the patients in showing positivity during heart treatment,” Mittal told IANS.

The United Nations has declared June 21 as International Yoga Day in a global endorsement of the ancient Indian discipline.

A large number of Yoga mats will be spread along Rajpath and the verdant lawns of the Central Vista at India Gate in the national capital on Sunday (June 21). Prime Minister Narendra Modi will lead the nation in some early morning Yoga exercises – with 192 countries across the world joining in.
